The DAISY Award honors and celebrates the skillful, compassionate care nurses provide every day.
The DAISY (Diseases Attacking the Immune System) Foundation was established by the family of J. Patrick Barnes after he died from complications of the auto-immune disease ITP in 1999. During his hospitalization, the Barnes family deeply appreciated the care and compassion shown to Patrick and his entire family. When he died, they felt compelled to say “thank you” to nurses in a very public way.
The DAISY Award is an international recognition program that was developed to honor nurses for demonstrating clinical skill, compassionate care, exemplary service and a commitment to excellence.

Nominate a Nurse
To nominate a nurse (RN or LPN) at Tomah Health as a deserving recipient of The DAISY Award, please complete the online nomination form. Please include why the nurse is extraordinary or share a story about why the nurse is so special.
Nurses may be nominated by patients, families, visitors and colleagues. Award recipients are selected by a committee of nurses at Tomah Health.
